What does a NDIS plan look like?

Each of the NDIS plans differs according to the individual and dependent on your own needs. Your NDIS plan fits into your personal situation, goals, supports and funding. The National Disability Insurance Agency (NDIA) will periodically change the appearance of the NDIS plan, however the provided examples here are consistent with the topics and subtopics used by the NDIA.

About Me

This information is about a participant name Zac and his entire life.

 

Zak James lives at 23 Excel Rd, Suburbia, Victoria 3096 with his parents, Kathy and Sam. He depends on his parents for most of his bills. His parents work part-time and help with transport to Day Activities. He regularly attends a Day Program that slated on weekdays between 9am and 3pm and spends his weekend with his parents. Zak needs support to relate with the community and needs absolute supervision for safety. Zak has limited friends apart from parents. He also possesses a limited interpersonal skill, sensory processing and related attitudes are his challenges.

Part 2

My Goals

This section deals with my Participant’s primary goal and other import things I’ll be working on.

My first goal is: To improve my home life.

In the course of this plan, I want: help with my personal care 3 days a week. NDIS to help fund the skill acquisition program to improve my independence. NDIS to fund respite to sustain informal support.

My Second Goal is: To safely access a variety of daily activities

During this plan I want: NDIS to fund Day Program 3 days a week. I also need a behavioural assessment and management plan to assist others and myself with strategies to attend daily activities. I need funding for assessment and intervention re-environmental situations.

My Third Goal is: To journey independently to community activities

During this plan I want: a direct assistance to access community activities once a in thirty days. I also want training to improve my knowledge and skills in mobility. I want funding for transport allowance for independence in travelling.

My Fourth Goal is: To extend life skills

During this plan I want: help worker in training to increase independence in daily living skills such as cooking and reading.

My Fifth Goal is: Improve communication and social skills

During this plan I want: speech therapy assessment, ongoing intervention and aids to enable improved communication.

My Supports

These are the supports that will assist and guide me to work toward my goals.

Informal support (Friends and family)

Mother and father: Daily assistance with all activities such as interaction, accessing the locality, decision making and finance maintenance.

Service and Community groups
(Mainstream and Community Support)

According to the NDIS, this is called “community and mainstream” supports. It includes health supports, schools or education services, sporting or hobby clubs, community groups or other government services.

NDIS reasonable and necessary support budgets

You can determine how you spend the amount in each budget highlighted below by reviewing the prices of the NDIS and matching it with the NDIS website participant page www.ndis.gov.au/participants.

Wherever a support is listed as “stated”; you have to purchase the support as explained in the plan. You must not modify a “stated” support on the list.
