How to Refer
Referring a client is simple and straightforward. If you are an NDIS registered provider, please fill out our secure referral form, which allows you to provide essential information about the client’s needs and goals. For family members, you can fill the form online, download the form, fill it out and send it to our postal address Suite 312b, 84 Hotham Street, Preston VIC 3072. In addition, there is a third option where you can reach out directly to our team via phone or email, and we will guide you through the referral process. We value open communication and collaboration, ensuring that all parties involved are aligned in providing the best possible support for the client.
