Location: Preston, Reservoir & Heidelberg
The primary purpose of this role is to support and empower people with disabilities so they can engage/take part in community services, mainstream services and for some to achieve desired outcomes as defined in their personalised plan. Our key aim is to work with participants and their families/carers to build capacity to manage their plan and supports independently.
Required Qualifications/Licences:
- Advanced Diploma Disability/Community Services (advantageous)
- Full registration with AHPRA
- Current Driver’s License and reliable Vehicle
- Current Working with Children Check
- Current Police Check
- Current First Aid Certificate
Essential Criteria:
- A driven individual with a passion for the Healthcare Industry – in particular, working with aged care and people with disabilities
- Self-starter and a team player
- Liaison and networking- to maintain current knowledge and build contacts for client referrals
- Service provision NDIS to work with the client to better understand and implement their NDIS plan accordingly
- Financial management/data Collection-monitoring outcomes and expenditure for client plan and maintain accurate and professional client records using Our database system of choice
- Contribute to ongoing development of Prime Assist Services and understand the suite of services offered
- Generate and maintain strong community links
Salary is negotiable based on qualifications and experience.
